Lyumjev (Insulin)

What is a Generic Drug?

A generic drug is a copy of the brand-name drug with the same dosage, safety, strength, quality, consumption method, performance, and intended use. Before generics become available on the market, the generic company must prove it has the same active ingredients as the brand-name drug and works in the same way and in the same amount of time in the body.

The only differences between generics and their brand-name counterparts is that generics are less expensive and may look slightly different (eg. different shape or color), as trademarks laws prevent a generic from looking exactly like the brand-name drug.

Generics are less expensive because generic manufacturers don't have to invest large sums of money to develop a drug. When the brand-name patent expires, generic companies can manufacture a copy of the brand-name and sell it at a substantial discount.

Description

Lyumjev is an ultra-rapid-acting insulin analog indicated for the management of blood glucose in adults and children with type 1 or type 2 diabetes mellitus. It contains insulin lispro-aabc, a modified form of insulin lispro, formulated to provide faster absorption and onset of action compared to traditional rapid-acting insulins. Lyumjev begins lowering blood glucose within approximately one minute of injection, with a peak effect at around 1 hour, and a duration of action of 4 to 5 hours. This makes it particularly effective for controlling postprandial blood glucose spikes. Its formulation includes specific excipients that accelerate absorption, offering more physiologic mealtime insulin coverage.

Directions

Lyumjev should be administered subcutaneously within 20 minutes before a meal or immediately after starting a meal. It can be injected in the abdomen, upper arm, thigh, or buttocks. Dosage should be individualized based on frequent blood glucose monitoring, meal size, and activity level. Lyumjev may also be used in insulin pumps (CSII) or administered intravenously under medical supervision. Patients should rotate injection sites within the same region to reduce the risk of lipodystrophy. Lyumjev is not interchangeable with other insulin formulations unless directed by a healthcare provider.

Ingredients

Each mL of Lyumjev contains 100 units of insulin lispro-aabc. Inactive ingredients include treprostinil sodium, citrate, magnesium chloride, zinc, glycerol, metacresol, and water for injection. The formulation is adjusted to a pH of 7.5 using hydrochloric acid or sodium hydroxide. The solution is clear and colorless.

Contraindications

Lyumjev is contraindicated in patients with hypersensitivity to insulin lispro-aabc or any of the excipients in the formulation. It should not be used during episodes of hypoglycemia. Intramuscular administration is not recommended due to unpredictable absorption.

Cautions

Blood glucose levels must be closely monitored during therapy initiation and dosage adjustments. Lyumjev’s ultra-rapid action increases the risk of hypoglycemia if meals are delayed or skipped. Dose requirements may vary in patients with renal or hepatic impairment. Switching between different insulin types or delivery methods should be done under medical supervision. Drug interactions with agents that affect glucose metabolism, such as corticosteroids, beta-blockers, or alcohol, should be closely monitored. Patients should be educated on proper injection technique, signs of hypoglycemia, and the need for emergency carbohydrates or glucagon.

Side Effects

Side effects associated with Lyumjev may include:

  • Hypoglycemia (most frequent)
  • Injection site reactions (e.g., pain, erythema, swelling)
  • Lipodystrophy at injection sites
  • Allergic reactions (local or systemic)
  • Headache
  • Nausea

Frequently Asked Questions about Lyumjev

How is Lyumjev dosed?

The dose depends on carbohydrate intake, blood sugar levels, and insulin sensitivity. Your provider will tailor your dosing schedule.

Is Lyumjev suitable for people with insulin resistance?

Yes. It can be used in insulin-resistant type 2 diabetes, but higher doses may be needed. It’s also available in U-100 and U-200 pens for flexibility.

What’s the difference between Lyumjev U-100 and U-200?

U-100: Standard concentration (100 units/mL)
U-200: Double strength (200 units/mL), helpful for high-dose users to reduce injection volume

Can I reuse needles with Lyumjev pens?

No. Always use a new needle for each injection to avoid infection, pain, and inaccurate dosing.

What should Lyumjev look like before injecting?

Lyumjev should be clear and colorless. Do not use it if it appears cloudy or contains particles.

What should I do if I overdose on Lyumjev?

Seek immediate medical attention. Overdose can cause severe hypoglycemia, which may lead to unconsciousness or seizures if untreated.

Does exercise affect Lyumjev dosing?

Yes. Exercise increases insulin sensitivity, potentially lowering your need for insulin. Monitor glucose more frequently and adjust as advised.

Can Lyumjev be used with a sliding scale?

Yes. Lyumjev can be used in correctional or sliding scale regimens, especially for tight post-meal control in type 2 diabetes.

Can I use Lyumjev if I have kidney or liver disease?

Yes, but dose adjustments may be needed. These conditions affect insulin clearance, so monitor blood glucose closely.

How should I dispose of Lyumjev pens and needles?

Place used pens/needles in an FDA-cleared sharps container. Never throw them directly in the trash or recycling.
